Employers:

To hire a new employee, these documents must be completed and submitted to Imagine Enterprises for further processing as well as the background check. To submit the document, either scan/email to Cheryl Harris at cheryl.harris@imagine-enterprises.org or fax to 866-672-6062. Once the documents are reviewed and the background check completed Cheryl will contact you with the employee start date.

Tips:

For the I-9 document, only print and complete page 7. Along with page 7, a copy of the applicant’s driver license and social security card needed. A US Passport is also an acceptable document.

If the employee wants direct deposit they need to complete the Direct Deposit Authorization form and include either a voided check or document from their bank that verifies their account and routing numbers.

If the employee will transport the individual a copy of their auto liability insurance needs to be submitted as well.

Anytime the employee wants to make changes to their tax withholding or address, they must turn in a new IRS W-4 form.
